Your health information rights

Complete Mind Care of PA is committed to protecting the privacy of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Pennsylvania law.

How we may use and disclose your information

We may use and disclose your health information for treatment, payment, and health care operations — for example, to coordinate your care among providers, bill your insurance, and run our practice. Certain disclosures are permitted or required by law.

Your rights

You have the right to inspect and request a copy of your records, request corrections, request restrictions on certain uses and disclosures, request confidential communications, and receive an accounting of certain disclosures. You may also request a paper copy of this notice.

Our responsibilities

We are required by law to maintain the privacy of your PHI, provide you with this notice of our legal duties and privacy practices, and notify you following a breach of unsecured PHI.
