What is Spravato?
Spravato (esketamine) is a prescription nasal spray FDA-approved for treatment-resistant depression and for depressive symptoms in adults with major depression and acute suicidal ideation. It works on the brain's glutamate system — a different mechanism than traditional antidepressants — which is why it can act far more quickly.
Because of how it works, Spravato is given in our office under the supervision of our care team. You self-administer the spray and are monitored for about two hours afterward. You will need someone to drive you home.

How it works
On the day: how a Spravato session works
Every dose is given in our office under supervision, exactly as the FDA REMS program requires. Here is what a treatment visit looks like, start to finish.

1. Check-in
You arrive and settle into a comfortable, private treatment space. A clinician confirms your vitals and reviews how you have been feeling.

2. Supervised self-administration
You self-administer the esketamine nasal spray yourself, under the direct supervision of our care team.

3. Two-hour monitoring
You relax while our team monitors your blood pressure and well-being for at least two hours, as required for safety.

4. Discharge with an escort
Once you are cleared, you head home with the driver you arranged. You must not drive until after a full night of sleep.
REMS enrollment and prior authorization
Because esketamine is dispensed through a federal Risk Evaluation and Mitigation Strategy (REMS) program, both you and our clinic must be enrolled before your first dose, and every session is supervised on-site. Spravato also typically requires prior authorization from your insurer. Our billing and authorization team manages the REMS paperwork and secures approval and your benefits verification before treatment begins — so the only thing you have to focus on is feeling better.

How fast does Spravato work?
For some patients, depressive symptoms improve in as little as 24 hours — much faster than traditional antidepressants. Response varies, and your provider will set realistic expectations.
Is Spravato safe?
Spravato is FDA-approved and given only in a certified, monitored setting precisely so it can be used safely. The most common short-term effects are dizziness, a dissociative or "floaty" feeling, mild nausea, or a temporary rise in blood pressure — which is why we monitor you for at least two hours after each dose.
Can I drive after a session?
No. Spravato can cause temporary dizziness or drowsiness, so you must not drive until after a full night of sleep. Arrange for someone to drive you home from every visit.
Is Spravato safe during pregnancy?
Spravato is generally not recommended during pregnancy or while breastfeeding. If you are pregnant, planning to become pregnant, or nursing, tell your provider — we will weigh the options and consider alternative treatments together.
How many sessions will I need?
Treatment usually begins twice weekly during the first weeks, then tapers to weekly and eventually every other week as you respond. Your provider personalizes the schedule based on your progress.
Is Spravato the same as ketamine?
Spravato (esketamine) is a specific, FDA-approved formulation derived from ketamine, delivered as a nasal spray in a monitored medical setting. That FDA approval is what makes it insurance-eligible for treatment-resistant depression.
Can I keep taking my antidepressant?
Usually yes — Spravato is most often used alongside an oral antidepressant. Your provider will review your full regimen and coordinate everything under one team.
